Registered Nurse - Case Management - Needed in Denver, CO.

Location – Denver, CO.
Practice Type- Registered Nurse - Case Management (2 Positions Open)
Start Date – As soon as credentialing is complete
Shift - Tuesday-Saturday 8: 00am-4: 30pm.
Shift - Sunday – Thursday   8: 00am-4: 30pm.
Term - 12 Week
Call - NO

We have an immediate need for two (2) Registered Nurses - Case Management - for a hospital located in Denver, CO. Below are additional details:

Requirements - Facility in Denver seeks F/T RN Care Coordinator for excellent contract working in Hospital Care Management department. - Must carry RNCC-RN License, BSN preferred. - RNCC candidates need to have at least 2 years of UM and/or discharge planning experience with MCG. - At least 2-3 years of clinical inpatient experience and EPIC experience required. - Coordination involves assessment, planning, support, and evaluation of patient care and related outcomes. - Activities to be performed are facilitation of appropriate status and level of care, screening for high risk and transition of care needs, - interviews with patient/family, progression of care facilitation with the healthcare team, performing insurance reviews and ensuring compliance with all state, federal and third party payors.
